Only comparing employees against one another and not evaluating against objective criteria
Failing to write down all the key points, goals, objectives and target dates mutually agreed upon
Unwilling to change ratings if called for in light of new information or reasonable circumstances
Praising employees when not justified by job performance
Worrying about de-motivating employees by telling them the truth
